Application Process
Applications are invited for eligible candidates residing within the municipal limits/jurisdiction of District Health Society Kupwara. The positions are contractual and temporary under National Health Mission (NHM).

List of documents to be uploaded with the application form:
i. Domicile Certificate.
ii. Date of Birth Certificate (Matriculation Certificate) from recognized University.
iii. Degree/Diploma certificate of requisite qualifications sheet indicating marks of all the years of the requisite qualification sheet for all years/ one consolidated mark sheet/ India/State Medical Council/J&K state Nursing and Marks sheet of all past applied for.
Experience Certificate duly attested by Head of the institution/department (if applicable).
vi. Under APMC Council attested by Director Health Services/domicile should be enclosed with Revenue Department Category for various Sub Centres of District), application forms unmarried Female candidates). Form under APMC certified issued by competent authority (Tehsildar). In case authority (Tehsildar) should be accompanied with Unmarried Certificate duly issued by competent authority (Tehsildar).

Instructions for applying Online:
- Candidates are required to apply online through kupwara.nic.in/application-form which is available on NIC Kupwara Website (2). No other mode of application will be accepted.
- Candidates are required to have a valid Email ID.
- The requisite original documents including domicile certificate issued by competent authority (Tehsildar) on prescribed format should be uploaded along with application form
- The candidate needs to ensure that all required fields are filled up with correct information before final submission
